Swift Support Coordination Au is a female owned business providing Support Coordination and Specialist Support Coordination services.
Based out of Cairns, Qld, our staff have the ability to support NDIS participants across the whole north of Australia from North Queensland, the Northern Territory and northern Western Australia. We’re your northern Australia specialists!
We can come to you in rural and remote areas periodically for face-to-face support, and the majority of our work can be done remotely via phone, video conference and email.
Our role is to ensure that your needs are adequately met with service providers local to you, who you know and trust.
What your Support Coordinator will do for you
- Help you to understand your NDIS Plan
- Break down your funding package to get you the most value
- Connect you to new providers
- Liaise with your existing providers
- Co-ordinate stakeholder meetings
- Advocate for you when your needs change
- Complete NDIA Reporting
- Attend planning and review meetings with the NDIA/LAC
- Answer your questions about the NDIS
Our staff are qualified mental health clinicians who can also provide Psychosocial Recovery Coaching.
We work with Self Managed and Plan Managed NDIS participants.

Specialist Support Coordinator
Hannah is Social Worker with 20 years’ experience working in the mental health and disability sectors. Having worked as a Program Manager for the local NDIS LAC Partner for over 2 years, she is highly skilled at understanding NDIS Plans and has strong networks in Far North Queensland disability sector. Hannah’s most recent experience as a Team Leader for the Qld Health Acute Mental Health Services makes her the perfect person to understand your mental health needs. Hannah is a member of the Australian Association of Social Workers.
